چکیده

Abstract Background Myocardial work (MW) estimation by pressure-strain loops using an implemented speckle tracking software has showed to estimate LV contractile properties overcoming the load-dependency limit of left ventricular (LV) global longitudinal strain (GLS). This proved useful particularly in clinical setting characterized frequent hemodynamic variations, such as heart failure and valvular disease. However, variation MW parameters across different stages mitral regurgitation (MR) its impact on symptoms never been investigated. Objective The aim this observational study was assess variations deformation chambers mild, moderate severe MR. Methods consecutive patients with MR were prospectively enrolled. Exclusion criteria were: chronic atrial fibrillation, prosthesis, previous cardiac surgery.Clinical evaluation, blood sample tests, ECG echocardiography performed. Speckle analysis completed myocardial performed offline. Patients then divided into groups first according severity. Differences among analyzed student T test (or non-parametric tests for non-normally distributed variables) predictors (as NYHA class ≥ 2) explored logistic regression analysis. Results overall, 120 enrolled (40 40 moderate, MR). GLS improved severity, while Global PALS reduced constructive (GCW) wasted (GWW) significantly MR, efficiency (GWE) a trend towards reduction higher grades index did not changed three (Fig.1). Among parameters, emerged predictor (R2=0.2, p<0.001) These results are explained pathophysiology mechanism attempted compensation volume overload increased contractility parallel disease progress, although low efficacy increasing stroke work; LA diastolic function have early which is associated occurrence symptoms. Conclusions accurately reflect pathophysiologic mechanisms burden Fig. 1 2

Left atrial function and deformation in chronic primary mitral regurgitation.

AIMS To study global and regional left atrial (LA) mechanics in chronic primary mitral regurgitation (MR) with echocardiography. METHODS AND RESULTS LA volumes during reservoir, conduit, and contractile phases were measured in 27 MR patients and 25 controls. LA ejection fraction (EF) and ejection force were calculated. Reservoir (SR-R), conduit (SR-C), and contractile phase (SR-A) strain rate...

Regional left ventricular deformation in primary mitral regurgitation

Introduction MVP is a common disorder affecting 2-3% of the general population. MVP patients with severe MR may remain asymptomatic for a number of years; however, LV systolic function may be impaired post-operatively, even among those with normal pre-operative ejection fraction (EF). Myocardial strain is the assessment of myocardial local deformation and may provide a more accurate assessment ...

VALVULAR HEART DISEASE How to measure severity of mitral regurgitation

Correspondence to: Paul A Grayburn, MD, Baylor Heart and Vascular Institute, 621 North Hall Street, Suite H030, Dallas, TX 75226, USA; [email protected] The recently revised American College of Cardiology/American Heart Association guidelines for valvular heart disease emphasise that surgery is now indicated for severe mitral regurgitation (MR), even in asymptomatic patients, provided tha...

Exercise-induced changes in mitral regurgitation in patients with prior myocardial infarction and left ventricular dysfunction: relation to mitral deformation and left ventricular function and shape.

AIMS The aim of this study was to assess the relationship between exercise-induced changes in mitral regurgitation (MR) and echocardiographic characteristics of mitral deformation, global left ventricular (LV) function and shape at rest and after exercise. METHODS AND RESULTS Forty consecutive patients with ischaemic MR due to prior myocardial infarction (MI), ejection fraction <45% in sinus ...

Peak mitral inflow velocity predicts mitral regurgitation severity.

OBJECTIVES Mitral regurgitation (MR) is a common echocardiographic finding; however, there is no simple accurate method for quantification. The aim of this study was to develop an easily measured screening variable for hemodynamically significant MR. BACKGROUND The added regurgitant volume in MR increases the left atrial to left ventricular gradient, which then increases the peak mitral inflo...
